Computer Based Quality of Life Study for Youth with Disabilities

What?

 One time computer-based study on the impact of health on quality of life during childhood. 

• Parent will be asked to complete a few demographic items and child will complete a short questionnaire about health,
• Survey should take around 20 minutes to complete,
• Each child will receive $10.

Where?

BOTH PARENT AND CHILD MUST BE PRESENT AT THE SAME TIME.  Survey can be completed:

• Online from your home (if you have a computer and access to the internet),
• At the UW campus,
• At Children’s Hospital.

Who?

 To participate:

• Child must be between 8 and 17 years old,
• Parent and Child both must be able to read and understand English,
• Parent and Child both must be comfortable reading from a computer and using a keyboard and mouse,
• Child and parent must be able to answer questions about child’s health.
